Vision and Hearing Permission Slip

Springs Charter provides vision and hearing screenings for all students in kindergarten, or upon first entry into a California school district, and in grades 2, 5, and 8 grades.  A color vision exam will also be performed, but only once on all male students and shall not be conducted sooner than the 1st grade.  Please review the vision and hearing schedule for available locations/dates. If your child is not in one of those grades, and you would like them to be evaluated, please fill out this form.
