Action Plan for Asthma

The asthma action plan informs school staff about a student’s asthma, the need for prescribed medication(s), and outlines the emergency steps to take in the event of an asthma attack. To be valid and implemented at the school site, the document must be signed by both the physician and the parent/guardian.

If the student is permitted by the physician to self-carry and self-administer medication, the student must also sign the second page of the action plan.

Although we encourage the use of our provided forms, a doctor’s office may use its own document. However, it must still be signed by the parent/guardian and the physician.
