Action Plan for Asthma

The asthma action plan is used for school staff to be aware of the student's triggers and emergency interventions that should be taken in case of a severe asthmatic reaction. The document needs to be signed by the physician and the parent/guardian acknowledging the doctors' orders for it to be valid and implemented at the school site.  Although we encourage our form(s) to be used, the doctor's office may use their own document/format, but must be signed by both MD and parent/guardian.
