Release of information for Health Services

This document allows Springs Charter Health Services department to communicate with the student's primary care provider(s) to determine appropriate care and eligibility for specific diagnoses. If this form is utilized to obtain documents pertaining to mental health, sexual health, and/or drug and alcohol abuse treatment, consent from the minor (if age 12 or older) is also required.
