Vision & Hearing Waiver form in English and Spanish

All students in kindergarten, or upon first entry into a California school district, and in grades 2, 5, and 8 grades will be screened for vision and hearing, as well as any students that have Initial Assessments and Triennial Assessments coming up for IEP purposes. This form should only be completed and turned in if you do not want your child included in the Vision & Hearing screening.
