Vision and Hearing Request Form

Thank you for coordinating your student’s health screenings. This form is used to request vision and hearing screenings for students with upcoming Initial or Triennial IEPs.

Please ensure you have confirmed the parent's intent to attend before submitting this request. Once submitted, our V/H Technician will coordinate scheduling. Please note that results will be uploaded to SEIS (Present Levels) and OASIS—we kindly ask that you check these systems for results before contacting Health Services.
